Toolbox Talk Record Template
v1.4
Purpose. Supervisors use this template to plan and record a toolbox talk so that consultation with workers on safety topics is documented.
1.Purpose of toolbox talks
Toolbox talks are short, regular safety conversations held in the workshop, the parts warehouse and the yard. They are the main way Torque consults workers on hazards, incidents, changes and procedures, as the Work Health and Safety Act 2011 requires.
2.Planning the talk
Choose one topic, keep it to 15 minutes and base it on a recent incident, a seasonal hazard, a new procedure or a question raised by workers. Bring the relevant procedure and any equipment you want to demonstrate.
3.Running the talk
State the topic and why it matters, demonstrate or explain the safe method, ask workers what gets in the way of doing it that way, and agree actions. Record every question and concern, even those you cannot answer on the spot.
4.Recording attendance and actions
Every attendee signs the record. Actions must have an owner and a date and must be transferred to the Incident and Hazard Register where they relate to a hazard. File the record in the safety folder within two days.
5.Follow up
The supervisor reports back on actions at the next talk. Workers who missed the talk must be briefed individually and sign the record.
